A five-month-old baby is fighting for life following a serious crash in northern NSW.

Just before 1.30pm on Monday, a car was travelling along Wallanbah Road at Firefly, about 36km west of Forster, when it left the road and collided with a fence.

The impact of the crash caused the vehicle to flip.

The driver, a 27-year-old woman, and her 46-year-old female passenger suffered serious injuries.

The pair were treated at the scene by paramedics before being airlifted to John Hunter Hospital in Newcastle in serious conditions.

A baby boy was also flown to hospital after suffering significant injuries in the smash.

He remains in a critical condition.

A crime scene has been established and will be examined by specialist officers from the Crash Investigation Unit.

Anyone who may have witnessed the crash or who has dashcam vision from the area is being urged to contact police immediately.
